• How much does the person weigh? 

• Do they use a walker/rollator/cane?

• Do they have their own wheelchair/electric or manual?

• If no, do they require our equipment? (Do they need to lay down or can they sit up)

• Do they require assistance? One or two assist (for stand and pivot) Or do they need transferred over (total lift)

• Date of transport? 

• Pick up time? 

• Return pick up time? 

• Duration of appointment or event? 

• Pick up address? 

• Destination address? 

• Are there stairs at either location, if so, how many? 

• Are they on oxygen?

• Any contact precautions? 

• Will there be a ride along? (The ride along must be self-sufficient and please provide their weight.)
